By accessing or using the Vertex LLC electronic health records system and related services, you agree to be bound by these Terms of Service. If you do not agree, do not use the system.
Vertex LLC provides outpatient substance use disorder (SUD) and mental health treatment services, including but not limited to individual counseling, group therapy, medical assessments, and care coordination. Our electronic health records system supports clinical documentation, scheduling, billing, and communication.
By opting in to receive SMS/text messages from Vertex LLC, you agree to the following:
Staff accounts are assigned by program administrators. You are responsible for maintaining the confidentiality of your login credentials and for all activity under your account. Notify your administrator immediately of any unauthorized use.
You agree to use the system only for its intended clinical, administrative, and billing purposes. You may not:
All patient information is protected under HIPAA, 42 C.F.R. Part 2, OASAS Part 822, and applicable New York State law. Unauthorized disclosure of patient information is prohibited and may result in disciplinary action and legal consequences.
Vertex LLC provides this system on an "as is" basis. While we strive for accuracy and reliability, we do not warrant that the system will be uninterrupted or error-free. Vertex LLC is not liable for any indirect, incidental, or consequential damages arising from the use of this system.
We may update these Terms of Service from time to time. Continued use of the system after changes constitutes acceptance of the revised terms.
